A 26-year-old Delhi man undergoing infertility evaluation was found to have a uterus and fallopian-tube-like structures due to rare PMDS.

The man visited RG Hospitals in Rajouri Garden, Delhi, after experiencing difficulty conceiving with his wife. Medical investigations showed that he had azoospermia, meaning no sperm were detected in his semen.

Doctors also found that both of his testes had not descended into the scrotum. Further examinations, including ultrasound and MRI scans, revealed a uterus-like structure and tubular structures resembling fallopian tubes inside his abdomen. Genetic testing showed a 46,XY chromosome pattern.

New Delhi: A rare medical case has emerged in Delhi after a 26-year-old man undergoing infertility evaluation was found to have a uterus and fallopian-tube-like structures inside his abdomen. Doctors diagnosed the patient with Persistent Müllerian Duct Syndrome (PMDS), a rare congenital condition.

What Is Persistent Müllerian Duct Syndrome?

Persistent Müllerian Duct Syndrome, or PMDS, is a rare congenital condition in which Müllerian structures, including the uterus and fallopian tubes, remain present in a person with typically male chromosomes and male external development.

During typical male fetal development, the Müllerian ducts regress under the influence of Anti-Müllerian Hormone (AMH). Problems involving AMH production or its action can result in these structures remaining in the body.

Link Between PMDS and Infertility

PMDS can be associated with undescended testes and infertility. Testes that remain inside the abdomen can have impaired sperm production, while the condition can also increase the risk of complications affecting reproductive health.

Doctors said the Delhi case was particularly unusual because the condition remained undiagnosed until adulthood and was discovered during an infertility investigation.

The case highlights why unexplained male infertility and undescended testes may require detailed medical evaluation, including imaging, hormonal testing and genetic investigations.
